Your First Visit


Just as there are many factors that can cause infertility, there also are many factors that can affect your chances for successful infertility treatment including age, your personal diagnosis, duration of infertility, and the number of previous unsuccessful treatment attempts. In addition, chances for your success will be influenced greatly by the specialist involved with your care.

This important meeting provides the direction for your future care. You will meet with Dr. Goldstein who will review your medical history, discuss your options and may even perform an ultrasound and/ or a physical exam. You will be introduced to Linda, our fertility nurse specialist, who will help coordinate the testing and will review your treatment plan recommendations. Finally, our office manager, Michelle will review your insurance benefits and financial options, and also provide you with information about the cost of treatment.

And it's not just about the woman! Both members of the couple count and together are an important part of the treatment equation. We rely on you to provide us with complete and accurate information about your history with infertility and to communicate with us openly while you are in our care. That is why we strongly encourage both partners to attend the initial consultation. Please bring your driver's license and insurance card(s) with you.

Whether you are already diagnosed or just beginning to explore treatment, you can be confident with a patient-centered, comprehensive approach to diagnosis and treatment.

Before You Come
To make the most of your consultation, we ask that you submit the following materials prior to your first visit:
  • Please fax or mail an enlarged copy of the front and back of your insurance card(s).
  • All previous infertility medical records including lab work, any prior gynecological surgeries, your most recent pap smear, HSG films and your partner's semen analysis
  • You will receive in the mail a letter which includes the initial female and male patient history form. Please complete and then bring this with you for your consultation or you may download this form by clicking patient questionnaire link at right.
